Survey on End of Life Care services for Brent patients/carers

Brent Clinical Commissioning Group (CCG) is working to deliver joined-up care for people at the last phase of life in Brent and want to hear from patients and their carers of their experiences of these services

The Pembridge Hospice in-patient unit has been suspended since 1 October 2018 due to a lack of palliative care staffing with no prospect of reopening.  We currently have a range of End of Life Care (EOLC) services provided by 4 different hospice providers.  Services are fragmented and there are variations in levels of quality, access and availability.  The closure of the Pembridge unit provides NHS commissioners with an opportunity to review all EOLC services and consult with local patients, carers and partners to develop a more joined up service for Brent patients.

We would be grateful if you could share your views with us as part of this process by completing the following questionnaire.  Please note this survey will close at 5pm on Friday 19th April, 2019.
