Within the Deaf community there have been regular incidents of communication breakdown between the patient and their GP/GP surgery.
 
We are asking you to fill in this survey so that we can identify where the breakdowns are and work with you to ensure that both GP's and patients understand how to support surgeries to commit to their legal obligations under the Accessible Information Standard policy (2016)
