Cyflwyniad


Mae’r arolwg am wasanaethau iechyd meddwl pobl ifanc yng Nghymru trwy gyfrwng yr iaith Gymraeg.

Rydyn ni eisiau clywed profiad pobl ifanc am gael mynediad at wasanaethau yn y Gymraeg, a pha mor ddefnyddiol roedden nhw. Mae hyn yn cynnwys pobl dan 18 mlwydd oed, neu gyda phrofiad diweddar am geisio derbyn gwasanaeth.

Mae’r wybodaeth yn helpu ni creu darlun mawr o wasanaethau iechyd meddwl plant a phobl ifanc ar draws Gymru er mwyn i ni lunio argymhellion ar sut gallan nhw wella.

Os rwyt ti’n rhiant, gofalwr, athro neu unrhwyun gyda phrofiad cynorthwyo person ifanc trwy’r system yng Nghymru, hoffwn glywed eich profiad.

Mae cymryd rhan yn yr arolwg hwn yn wirfoddol. Bydd eich atebion yn ddienw – fydd neb yn gwybod pwy sydd wedi dweud beth. Gallwch chi stopio ar unrhyw adeg ac os nad ydych chi’n dymuno ateb cwestiwn, does dim rhaid i chi.

Bydd canfyddiadau’r arolwg hwn yn cael eu rhannu ag eraill gan gynnwys gweithwyr Mind, pobl sy’n gwneud penderfyniadau am wasanaethau a chymorth iechyd meddwl a'r cyfryngau. Ond ni fydd neb yn gwybod beth yw eich atebion unigol chi.  Peidiwch ag ysgrifennu eich enw neu gyfeiriad.

Introduction

This survey is about Welsh language mental health services for young people who live in Wales.

We want to hear from young people about experiences of accessing services in Welsh, and how useful you found those services. This includes young people under 18 now or with recent experience of trying to access services.

The information which you provide will be used by us to build up a picture of Welsh language mental health services for children and young people in Wales so that we can make suggestions for how they can be improved.

If you are a parent, carer, teacher or have otherwise supported a young person through the system in Wales, then we would also like to hear about your experiences.

It is up to you whether you want to take part in this survey. All answers will be treated in confidence. If you start the questionnaire and then change your mind about participating, you can stop at any time.

The survey findings will be shared with others – including researchers, policy makers and the media. However, your answers will remain anonymous – no one will know who has said what. Please do not write your name or address on this questionnaire.
