This survey relates to the chronic pain services provided by the Royal National Hospital for Rheumatic Diseases (RNHRD) Bath Centre for Pain Services (BCPS). These services are located at the Mineral Water Hospital in Bath. You do not need to have been a patient of this service to take part in this questionnaire. The survey will take around five to ten minutes to complete.

The Bath Centre for Pain Services provides pain rehabilitation services for people experiencing chronic pain.

We're proposing to relocate these services to a specially refurbished building on the Royal United Hospitals NHS Foundation Trust (RUH) Combe Park site and we’d like to hear your views.

Patients will continue to be seen and treated by the same team, providing the same high standards of care.  The service will still be located in Bath. The new location will provide therapy and group rooms, residential accommodation and a dedicated therapeutic courtyard area, all located in  a quiet setting, away from the main acute hospital building.

This is part of a careful and phased approach to relocating all RNHRD services provided from the Mineral Hospital site to the RUH or appropriate community based settings.

We welcome your views about how you would like to see these services provided now and in the future. Your feedback will help us in our planning to ensure we continue to provide the best service for our patients.

Please be assured the information provided by you in this questionnaire will not be used in a way which would allow identification of you as an individual.
