The purpose of this survey is to help you understand more about your habits and the drivers behind these habits.

It should take approximately 6 minutes to complete.  

Many of the questions examine your deeper motivations and behavioural influences. It is vital that you are completely honest in your responses to these questions as this will have an effect on your progress towards your goal.

I will analyise your results and send you your personalised Habit Report. This report will highlight key areas of focus that will help you move towards establishing healthier habits that last.
