WELCOME TO THE CARER SUPPORT SURVEY

What is this survey about?
This survey asks about the experiences and views of people caring for or supporting people who are living with dementia. We would welcome your help if you are a carer who lives in East Lothian of if the person you care for or support lives or used to live in East Lothian.  We use different terms as people describe what they do and their relationships in different ways. 

The information you provide will be used to develop a Carer Strategy for East Lothian. This will be required by law in recognition of the important and essential role that carers play.

To help us interpret the survey,  we have asked for some information about you at the end of the survey. This information will not enable us to work out who you are. You only have to answer the questions you choose to.

Who should fill it in?
Please complete this form if you or the person you care for/support currently or used to live in East Lothian.   You do not have to live with or be related to the person you support. We know that neighbours, friends and local organisations and services provide important support in the community. 

If you have previously cared for or supported someone with dementia, your experiences and views will be very helpful for present and future carers.

If your paid work is to provide care and support, please do not complete this survey. We value your work but this survey is specifically about unpaid carers.

What if I care for or support more than 1 person?
The survey is about you and your role as a carer and the support you get. So please answer based on your whole experience of caring for people with dementia. Please do not include other caring experience (e.g. childcare). 

What if I don't care for or support someone with dementia?
If you are not or have never been a carer for someone with dementia, but are interested in finding out about dementia or getting involved, there are links to various websites that you might find useful at the end of the survey.  You can also leave us your contact details so we can contact you.

What if there has not been a formal diagnosis of dementia?
The person you support/care for does not have to have a formal diagnosis of dementia, we know that many people with dementia do not have a diagnosis from their GP, and we want to ensure that everyone is included. Please complete the survey if you think the person might have dementia.

Will my answers be kept confidential? 
The information you provide is anonymous and we will ensure that no individual can be identified in our analysis and reporting.

What happens next? 
If you would like to tell us more about your experiences, please add your contact details at the end of the survey and we will get in touch.  We will delete this information before we analyse the survey.

We will be using the findings to help the Council develop its support for carers.  We will share the report widely, if you would like us to send you a copy, please leave your contact details at the end of the questionnaire.

The survey has been funded by the East Lothian Health and Social Care Partnership.

The Survey will close on 2nd May 2016.
