The Get Involved Group wants to hear your views on out-of-hospital support. By Out-of-hospital support, we are referring to the practical and emotional support that would help you get back to a regular routine and into the community after an operation or a longer stay in hospital.

We’d like to know if you’ve ever received out-of-hospital support and what your experiences were, if you’re aware of ways to get it, how it impacted you and what your ideal out-of-hospital support situation would look like.

Equally, we want to hear from you if you’ve never been in receipt of such support and how the lack of it impacted you.

Only share as much as you’re comfortable with. Please be aware that all responses will be handled confidentially and will be anonymously fed into our report which will be submitted to the Clinical Commissioning Group by the end of March 2020 with recommendations collated from your responses, which would help improving out-of-hospital support across the city.

If you need this survey in a different format, please contact us on 01273 20 89 34 or e-mail gig@possabilitypeople.org.uk
