1. INTRODUCTION AND GENERAL QUESTIONS

Carers Leeds would like to hear from you if you are, or have recently been, an unpaid carer in Leeds. We want to better understand your experiences and needs.

In this survey, we ask about your health and wellbeing, money and work,  social life and time for you, and access to services. Carers Leeds will use the results from this survey to help improve the lives of unpaid carers in our city. We are repeating this survey every year.

We know you may be busy and have other commitments, but we would be grateful if you could answer as many questions as possible. It would be great if you could answer all the questions but you can skip any questions you prefer not to answer. It should take no more than 20 minutes to complete the survey.

The survey is confidential, and you do not have to give your name. If you do share your contact details, this information will not be shared with anyone outside of Carers Leeds. Carers Leeds will publish the anonymised results of this survey.

Your responses are very important to us.
Thank you for taking the time to complete this survey.
