The purpose of this survey is to understand what people in Kingston upon Thames know about the COVID-19 NHS Test and Trace service and how it might need to be adapted to meet our local needs.

This survey will take you approximately 5 minutes to complete. All responses are completely anonymous. We will not ask for anything that identifies you individually, but what you tell us will be analysed and shared with stakeholders such as the NHS, Royal Borough of Kingston upon Thames and Healthwatch England, to help improve the response to Covid-19.

Your information will be used and stored for the purpose of this project, and in accordance with the Healthwatch Kingston upon Thames’ Privacy Policy which can also be provided in paper form on request via this email info@healthwatchkingston.org.uk. This report will be sent to local health and social care decision makers to help improve NHS and social care services.

We will publish this report on our Healthwatch Kingston website and Healthwatch England who use the anonymised report findings in their national work.
