Welcome back!
Welcome back to the P4P Collaboration Toolkit. Once again, to make best use of this resource please refer to the written guidance.
This survey will ask you to self-evaluate your progress against a range of factors that demonstrate best practice when it comes to developing a collaborative project. By determining your level of progress, this will allow you to focus on the areas where you are weaker and allow us to support you better through the process.
Every question requires an answer so where you are unsure, or if a question is not relevant to your particular situation, you should select the first option 'Not Started Yet'. It is also important that organisations complete the survey individually (not on behalf of other organisations within the collaboration) so that progress can be benchmarked across the members of the collaborative project.
The survey will take approximately 15 minutes to complete.
Having completed the survey previously, we will use your answers this time around to measure your progress and identify any further support needs.
