This survey aims to understand personal and organisational experiences of living with a serious illness or grieving in the workplace.  The hope is that by sharing both good and more challenging experiences we can support employers and colleagues in better understanding their unique contribution to wellbeing.

The use of this data will be anonymised; if however, you are happy to be contacted about your responses, please input your email in the form below.  A report, article and presentation will be produced from the data collected via this survey as part of our work with employers and communities.  No personal data will be stored or used for any other purpose.
