1. Consent Form

You are invited to participate in a web-based online survey on undergraduate student wellbeing. This is a research project being conducted by Dr Hugh Grant-Peterkin, Dr Catherine Marshall and Prof Ania Korszun at Barts and the London School of Medicine and Dentistry.  It should take approximately 7 minutes to complete.

PARTICIPATION
Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in this research survey, you may withdraw at any time. If you decide not to participate in this study or if you withdraw from participating at any time, you will not be penalised.

CONFIDENTIALITY
Your survey answers will be sent to a link at SurveyMonkey.com where data will be stored in a password protected electronic format. Survey Monkey does not collect identifying information such as your name, email address, or IP address. Therefore, your responses will remain completely anonymous. No one will be able to identify you or your answers, and no one will know whether or not you participated in the study.

At the end of the survey you will be asked if you are interested in participating in a prize draw. If you choose to participate you will be asked to send a separate email to an address provided at the end of the questionnaire. You will be asked to send an email so that your responses remain anonymous.
