* 8. Telephone Access - How do you rate your experience of contacting the surgery via telephone.

* 9. Text messaging has been available to remind patients of appointments for over 12 months and we are planning on extending the use of text messaging to remind patients who are due regular health checks.   Please indicate your views on this service.

* 10. We installed a patient check in screen in the waiting room in November to improve patient experience at reception.  
We envisage this will give reception more time to answer the phones and deal with patient queries in a more confidential environment. We are encouraging patients to use the check in screen and would like feedback on your experience or views on this additional facility.

* 11. Self Referral - Some services such as Mindsmatters and Antenatal are available via self referral.  Would you like to be able to self refer to other services such as Joint Health which includes physiotherapy services?

* 12. Patient Participation Group - We have an online patient group which we consult from time to time and always welcome new patients to join.  Please let us know if you would like information about the group or would like a registration form to join.

* 13. Health advice Apps - West Lancs CCG are launching a free website where you can search and compare health apps and download free to your phone, so you can find the right app to keep you healthy.  The website is:  westlancs.orcha.co.uk
