1. Introduction

This survey is for parents and carers in Suffolk who have children and young people aged 0-25 years with any additional need or disability (diagnosed or not). If you live in Suffolk or your child/young person receives services in Suffolk then you are entitled to complete this survey.  Your child/young person does not need to have an Education, Health and Care Plan in order to take part in this survey. Our aim is to see what the experiences of families has been in relation to education, health and care services during Covid-19.

The survey has two periods of time it is focusing on:
1. Lockdown between March and July 2020
2. The return to education or training from August 2020 to now.

This survey will help us to reflect the views of a wide group of parents and carers and should take no more than 20 minutes to complete.  Please complete the survey once for each child/young person you have with additional needs and/or disabilities i.e. if you have 3 children you will need to complete the survey 3 times reflecting the individual experiences for that child/young person.

Anonymised data will be shared with relevant education, health and care commissioners and providers within Suffolk as well as voluntary organisations and will enable us to show what has been working well as what needs to improve. We also use some of the individual comments to expand on and emphasise the survey findings, however, we also anonymise these. No information which would identify individual children/young people or families will be shared unless we have your express consent to do so.  We will also make public the survey findings.

We have asked you for your contact details so that a member of the SPCN team can contact you to find out further details about your experiences if we need to.  It would be really helpful if you would allow us to do this as it is sometimes difficult to say everything you need to about your experiences in a survey.  Your contact details will not be shared with anyone except the SPCN team.

We are also collecting equality and demographic data which is deemed to be personal sensitive data in relation to the 2018 Data Protection Regulations. As such, we need to provide you with our privacy statement that can be found here.

The SPCN team are sending out family support boxes for children/young people with any additional need and/or disability, (diagnosed or not) and their siblings and also contain information for parents and carers about how to support children and young people and other support organisations that are available. These boxes are free, personalised to each family and can contain a range of items including sensory bags, stress balls, bubbles and activity books and available to any parent / carer / guardian / foster carers who are members of SPCN.
