We would love to hear your views

This survey is for parents and carers, in Dudley, of children and young people aged 0-25 years with SEND (Special Educational Needs and Disabilities).

Over the next couple of years Inspectors from Ofsted and CQC (Care Quality Commission) will be inspecting the Borough Council and Health services in Dudley to see how well they identify children's special educational needs and/or disabilities, how well they meet those needs and how effectively they improve outcomes for those children and young people.  The views of parents are a very important part of the inspection and so they will be meeting parents on their visits and also holding meetings with parent representatives including Working Together For Change as the parent carer forum.  We do not know how soon the inspection will take place, it can happen at any time with just a few days notice. We are asking you to complete this survey now for two reasons, A) So that inspectors really know what life is like for families in Dudley and B) To ensure that the Parent Carer Forum members can continue to work with the Local Authority and Health to improve services and communication.   This survey will help us to reflect the views of a wide group of parents.  The survey should take no more than 5 minutes to complete and the information you give us is confidential and anonymous.

Please answer the following questions about how things are at the moment for your child or young person. The SEND reforms became law in September 2014 so please think about things from that date onward and not before then.

If you have more than one child with SEND (special educational needs and/or disabilities) then please complete a new survey for each child/young person.

If your child/young person does have any input from Health or Social Care about their SEND then please tick the Not applicable box.
