Finding out more about your views of what we do, what you need and who you are will really help us to shape our future plans and thinking – so many thanks, in advance, for taking time out to respond.
The survey is completely anonymous. It will take approximately 15 minutes to complete. Please don't worry if a question doesn't appear to be relevant to your situation or you feel unable to answer it - just respond to questions where you can, leave any that you're unsure about and move on.
As an added incentive we’re offering people who complete the survey the opportunity to enter a prize draw to win a £25 gift voucher. At the end of the survey there is a link to a separate form for you to submit your email address, to ensure this and your survey responses are entirely separate.
