What would you call a building that: brings together lots of different NHS, social care and services in one place. This will join-up the services you need, rather than having multiple appointments in different places. This is especially important for older people, and people with one or more life-long illnesses.
A range of services will be on offer within the building, including GP services, mental health and social care.
The joined-up care provided within these buildings will help to keep people as well as possible; avoid unnecessary hospitals visits and; get people home from hospital quicker as soon as they are well enough.
