Dear colleague,

I am the consultant engaged by Macc and Manchester Health and Care Commissioning to carry out a brief evaluation of the Community Explorers work in the city.  I work as an organisational development consultant, mainly for the voluntary and community sector in the Greater Manchester area.

The Community Explorers partnership provides an opportunity for the voluntary and community sector to work in partnership with Manchester Health and Care Commissioning in health and social care service development. Manchester Health and Care Commissioning was formed on 1 April 2018. It is a partnership between NHS Manchester Clinical Commissioning Group and Manchester City Council. The organisations came together to ensure a more joined up and effective approach to commissioning health and social care services for the people of Manchester.

You may have been involved with Community Explorers on a locality level (north, central and south Manchester) or at a city-wide level by attending one of the annual city-wide events (Locality Plan, Population Health Plan, working with Manchester Local Care Organisation).

The questionnaire asks you about this, so we can get an idea of who has responded to the survey. Please answer the main questions as fully as you can so we get a good understanding of your experience and views. Your feedback will be used to develop Community Explorers over the next year and how it can help inform and influence changes in health and social care taking place across the city.
 
I will be writing an anonymised summary report of the findings of the evaluation, so please do be open about your experience.
