Thank you for taking part in this survey about the draft Workforce Disability Equality Standard (WDES) metrics.

This survey is intended for you if you are a member (speaking on behalf of the network), lead or Chair of a:

· Disabled Staff Network
· Disabled Union Member Network
· Other equivalent or similar Network

Please respond to the questions on behalf of your network.

More information about the WDES and the metrics, including the draft metrics have been sent to you in the e-mail which included the link to this survey.

This questionnaire should take around 20 minutes to complete. All responses will be treated confidentially.

We are asking for your network’s views about the proposed draft metrics and, in particular, your views about Metric 9, because this metric focuses on the voices of disabled staff in the organisation.

Please note, we are asking for your opinions on the draft metrics – how they are worded, and whether they will make a positive difference to disabled staff working in the NHS. We are not looking for your trust’s or organisation’s response to the metrics themselves in this survey.

Your responses will be collated and analysed, and the results and conclusions will form an anonymised report. If you want to see a copy of the final report, please contact Christine.rivers1@nhs.net.

If you have any questions, want to discuss the survey or be sent the survey in a different format, or if you would prefer to give your responses in another way (for example, by telephone), please contact Christine.rivers1@nhs.net.

The survey closes on Friday 17 November 2017.
