Thank you for volunteering to take part in this survey for Ovarian Cancer Action, Registered charity no. 1109743 (England & Wales) and SC043478 (Scotland).

What is the purpose of the survey?
The purpose of this survey is to understand the current state of genetic testing for BRCA mutations and Lynch Syndrome in the United Kingdom. The questions will cover topics surrounding any genetic testing experience you may have had, the medical teams you visited and any thoughts you may have on your experience.

We will also ask some questions about any preventative surgery you may have had, and fertility options you may have been offered, though it is possible to skip these questions if you are not comfortable answering them.
Who is eligible for the survey?
Anyone in the UK, male or female, who has any experience of trying to get a genetic test, has had a cancer diagnosis, or already knows they have a genetic mutation is eligible.
What will this information be used for?
The information we gather from this survey will be used to evaluate how and where genetic testing for hereditary breast/ ovarian cancer is happening in the UK. The data will inform and drive our campaigning for the next year and shape our 2017 “Acting on BRCA” policy.
