Introduction

Aims for the Integrated Care System (ICS) Primary Care (PC) Advanced Practice (AP) Network:

To promote collaborative working
Educational and supervisory support for the advanced clinical practitioners
Promote effective and safe practice within the ICS

The purpose of this questionnaire is to gain consent of individuals to join the ICS PC AP network, with the added gain of gathering some relevant data that will guide how we support people in their roles.
