1. Survey Overview

Innervate is currently developing an 'APP' and we would really like as many people as possible to answer this short questionnaire (estimated to take approx 5 mins). Please note that with the first question, if this doesn’t apply to you just type in N/A. Do likewise for any other linked questions.

The app is being developed in the hope it can help to acknowledge and support the message that recognition of emotions resulting from either a physical or mental trauma is a significant and key step in supporting the health and wellbeing of individuals. 

All responses will be treated with respect. Thank you for taking the few minutes to complete the survey. If you wish to learn more about Innervate, you will be able to read more about us and our work at www.innervate.co.uk

* 1. Whether or not you have experienced a trauma in your life, we are keen that you don't skip this question. For those who have been through such an event/experience in your life, which includes an indirect event ie. witnessing an event, or you or a loved one may have received news of a diagnosis for a medical condition/illness(es), please write the information (and the diagnosis, if applicable) in the box if you are comfortable with doing so. If you haven't had any sort of experience just put N/A. This information is important as it may relate to Q5.

* 4. If applicable, and you have been diagnosed and willing to tell us how long you have you been diagnosed, or you experienced or witness a trauma (which of course, may be the news of a diagnosis) please select the appropriate box, to say when it was. You can skip this question if N/A.

* 5. This is the main question that we would like everyone to complete. It may take you a couple of minutes to complete (thank you!).

We have provided a comprehensive list of 'emotion words'. From this list, please select, by clicking the box next to the word, 18 words (exactly) from this list you are most likely to use, which could/would help you to describe how you may feel about any aspect of your life (not necessarily now, but at any point). A good range of emotion words (from positive to negative) within that selection is what we are interested in.

We will then look at the most frequently selected words from all the responses we get, and incorporate that into the design of the app. But don't worry, other words will be made available as part of a 'bank' of words for the user to access in the App.
