All professionals working with Children and Young People in Tower Hamlets are invited to take part.
 
The Tower Hamlets Child Healthy Weight pathway will clarify how our services can best work together to ensure children living with (or at risk of) excess weight are offered appropriate support that meet their individual needs.

This pathway highlights current roles and responsibilities of different workforce. It aims to support all professionals working with children aged 0-19 years around the topic of healthy weight by offering guidance on how to identify, assess, communicate, and appropriately support these children.
Aim of this consultation

The implementation of this pathway can only be effective if partners across the system work collaboratively and feel clear and confident about supporting children and young people around the topic of healthy weight. As you read the consultation you will be asked to provide:
1)      Your comment and feedback on the proposed pathway structure
2)      Your thought on how you will implement it within your own role or service

Please note that we recognise capacity might be a challenge, particularly amongst health services. This will be identified as a risk to delivery, alongside other gaps found in local support services.

The pathway consists of 8 headings set out on the image below. For the purpose of this consultation, we are only seeking comments and feedback in relation to headings 3, 4, 5 and 6 detailed in the following pages.
