Tell us about your experience

Trigger warning: this survey asks questions about experiences of death or serious harm in relation to the benefits system, which we appreciate may be distressing.

If you need to talk to someone you can call the Samaritans at any time, day or night on 116 123. If you are in crisis, you can contact your local crisis mental health team or GP, or dial 999 in an emergency. If you need advice and support on practical issues, please email our advice team at

While some cases have been reported in the media or investigated by the Government’s Department for Work and Pensions (DWP), we are concerned that they may be the tip of the iceberg.

We want to build a complete picture of benefit-related deaths and serious harm, to help make the case for a public inquiry and a new body to investigate future cases. An inquiry will also give families the answers they deserve, restore faith in the system and prevent further tragedies.

The evidence we collect here will also help us to identify specific problems within the benefit system and solutions that could make a difference.
