Project Development and Investment Officer 2021: Equality and Diversity Monitoring Form

The Sport and Recreation Alliance wants to meet the aims and commitments set out in its equality policy. This includes not discriminating under the Equality Act 2010, and building an accurate picture of the make-up of the workforce in encouraging equality and diversity.

We need your help and co-operation to enable it to do this. However, filling in this form is voluntary.

How will we use your data? Sport and Recreation Alliance will be the controller of your data, in full compliance with the Data Protection Act 2018.

We will be using Survey Monkey to collate the responses to the feedback survey. Responses may be given anonymously with no provision by participants of personal data. The definition of personal data within GDPR includes IP addresses of personal computers or devices; participants should be aware that if they choose to complete the feedback survey, Survey Monkey will store this data outside the EU in accordance with their own data privacy policy.
