This survey will act as a pre-requisite for football clubs and organisations before applying for support through the ‘Be Active Wales’ fund administered by Sport Wales that will be available from July 2020.
Please complete all relevant sections of the survey to provide us with as much information as possible on the impact of COVID-19 on your club. This information will help us review your club's current situation and future plans in order to assess whether an application through the Be Active Wales Fund would be appropriate. It will also ensure we have an accurate picture of the impact of COVID-19 on football communities, helping us to determine the appropriate future action to support the game at all levels across Wales.
