CREW CHANGE REPORT FORM

ICS is collecting information on the ability to conduct crew changes during the COVID-19 pandemic. Shipping companies or Masters are requested to complete the report form for both successful and unsuccessful crew changes (please complete a separate form for every crew change attempted). It should take about 1 minute to complete and is entirely anonymous (i.e. no information is required about the name of the respondent, company or ship).
