Why are we carrying out this survey?
Midlothian Health and Social Care Partnership is 'recommissioning' care at home services that are provided by external providers. This process is important because it decides what care at home services will do and how they will work. This process will lead to new contracts with providers for the coming years. We must undertake this process by law.
This survey is open until Friday 18th December.

We will use the information you give us to create a report. This will help us to decide what care at home services should do in the future.
