This survey is aimed at finding out more about the needs of
disabled artists and disabled people who wish to work in the creative sector


As you may know, Shape is 40 this year.  Whilst we want to celebrate this, we also need to make sure we can continue to provide support for disabled people. In order to do this, and get the funding required, we need to provide evidence, and be able to convey your views to the people prepared to make positive change happen.

With your help, we can continue to improve access, equality and inclusion.
