Patient and Stakeholder Proposed Merger Engagement Questionnaire

GP Practice Merger Proposal - Consultation Engagement is suspended until further notice, thank you for all your responses to date.  We will perform a new patient engagement process in the near future.


Your surgery would like to merge with the four practices listed below. By working together as one practice, we believe this will help us to provide improved access and clinical care.  We have been working together sharing our values and commitment to high quality clinical care over the past 2-3 years. This has been a hugely helpful and learning experience resulting in innovations such as Docly and better care planning for the more vulnerable of our patients. The 'new' practice would still have the same surgeries, same doctors - pooling their management and clinical teams would offer greater resilience and deliver a wider set of innovative services closer to patients. Our patients would have the choice to receive care across the merged practice sites.

What is the patient consultation about?
Answer: We want to listen to your views about the proposed merger of the GP Practices listed above.  We want to understand any concerns that you may have and address them before a decision is made.
How will you consult with your patients?
Answer: We plan to consult with patients in a variety of ways: Text message link to an online survey; a face to face drop in session at each surgery; hardcopy consultation questionnaire forms available at each surgery reception desk; posters in the waiting rooms; and details on our website.
How long will the consultation process take?
Answer: We plan to run a 45 day consultation process.
What will happen at the drop-in sessions?
Answer: You will be able to discuss any concerns or feedback with our staff face to face.  If we can’t answer a question at that point, we will respond to your query before the end of the consultation.
I would like to complete the questionnaire but would need to have it translated, is this possible?
Answer: If you have a certain communication requirement, we will be please to assist you, in order for you to engage with us.
How will my response to the questionnaire be used?
Answer: All feedback we be treated confidentially, and we will use the feedback to identify any concerns or issues as they arise.
If the practice merge goes ahead, when will it happen?
Answer: Hopefully mid 2020.
What will be different for me as a patient if the contracts merge?
Answer: You would notice no difference to your surgery.  No locations would close.  You would have the ability to attend the surgery of your choice across the Group.
