Patient Experience of Stroke |
Introduction
Thank you for completing this questionnaire which is about your experience of rehabilitation and care following a stroke. By listening to peoples experiences we can advise the people who commission and provide services on what can be done better.
As part of this work, we are also holding interviews with service users and carers, some members of staff, and people working across the Volunteer and Community sector.
The questionnaire will take around 30 minutes. There are no right or wrong answers – we are interested in your views and reflections. Some parts of this ask you to tell us about your care in your own words and also ask some specific questions to understand more about your experience, We will report all data anonymously, so no one will know who said what. A carer or friend may also complete this for you but please make sure they give your experience.
