The purpose of this questionnaire is to collect information as to why gundogs in the UK are retired/stop working. The information collected will be used to produce a publication in a scientific journal about the impact of health on the working life of gundogs. Your personal data (email address) will be held in accordance with data protection law and all information will be anonymised before publication. Your email address will be held on a password-protected database and only I will have access to it. It will be destroyed as soon as I am confident I do not need to seek clarification of any of the answers.

By completing the questionnaire and providing your email address you are giving your consent for the information provided in the questionnaire to be analysed and interpreted. If you wish to withdraw your data at a later date please contact me at john.houlton@btinternet.com
