Your views and experiences are important in helping the Local Authority to improve the transitions process for young people and their families. Thank you for your time in completing this survey which will help us improve our processes within B&NES so that all young people can experience a smooth and positive transition from children’s to adult services. 

Transition is when young people turn 18, and the services they access change from children’s services to adults. This means that if your young person had a children’s Social Worker, a Paediatrician, were accessing CAMHS or therapy services (Occupational Therapy, Speech and Language, Physiotherapy), these services would no longer be available, and they would need to access support through adult services instead.

Your young person will also have moved from secondary school to a post 16 setting (college, 6th form or onto an apprenticeship).

Before their 18th birthday, the professionals who work with them should have been starting to talk with you as a family about transition and what this might look like for you.
