General Information

The aim of this study is to track symptoms and monitor quality of life (QoL) changes across individuals in the UK who are at home because of the COVID-19 crisis.  We are asking you to complete an online survey examining daily life. You will be invited to join the study for the entire length of the pandemic and will be asked to complete an online survey atleast once every week.

The study will deliver a wealth of clinical data that will allow us to identify high risk groups (both susceptibility to infection and QoL decline), to better understand this novel disease and how self-isolation and quarantine can influence physical and mental wellbeing amongst a large population setting. The data gathered from this study will provide useful information for local councils and healthcare authorities to help with planning and prioritising    support to their local community during the pandemic.

We appreciate your interest in taking part in this questionnaire/online survey.   You have been invited to participate as you have taken part in other research studies, have agreed to be re-contacted, are above the age of 18 and have access to a smartphone or the internet.

You will be given some questions to read that are designed to track symptoms and quality of life changes.  It should take you about 20 minutes to complete, with a total of 54 questions.  No background knowledge is required.

Professor Ana Valdes and colleagues at the University of Nottingham will use this information to identify high risk groups (both susceptibility to infection and QoL decline), and to better understand the causes and spread of this novel disease and how self-isolation and quarantine can influence physical and mental wellbeing.

Do I have to take part?

Please note that your participation is voluntary. You may withdraw at any point during the questionnaire for any reason, before submitting your answers, by pressing the ‘Exit’ button / closing the browser.

How will your data be used?

Your answers will be completely anonymous, and we will use all reasonable endeavours to keep them confidential. 

Your data will be stored in a password-protected file and may be used in academic publications.  Your IP address will not be stored.  All questions are optional, or we have included ‘a Prefer not to say’ option for each set of questions if you prefer not to answer a particular question.

Who will have access to your data?

The University of Nottingham is the data controller with respect to your personal data, and as such will determine how your personal data is used in the study.  The University will process your personal data for the purpose of the research outlined above.  Research is a task that we perform in the public interest.  Further information about your rights with respect to your personal data is available from: https://www.nottingham.ac.uk/utilities/privacy.aspx/

We would like your permission to use your anonymised data in future studies, and to share data with other researchers (e.g. in online databases).

Any personal information that could identify you will be removed or changed before files are shared with other researchers or results are made public.

We believe there are no known risks associated with this research study; however, as with any online related activity the risk of a breach is always possible.  We will do everything possible to ensure your answers in this study will remain anonymous.  We will minimise any risks
