Healthwatch Together would like to hear your experience:

Lancashire and South Cumbria NHS Foundation Trust delivers a wide range of local services.

The Trust has asked Healthwatch Together to find out what is working well and less well so that they can improve what they do and how they do it.

We have all seen a lot of change this year. This includes the way we access and in some cases how NHS services are delivered.

At the moment they are working on a Clinical Strategy , your feedback will help to shape the approaches and patient experience for the future.

Before you start it’s important to know:

· Your views and options will be treated confidentially and anonymously.

.Information and feedback shared will remain confidential unless there is a need to address a specific safeguarding or quality issue. If this is the case Healthwatch Together will follow local safeguarding and escalation guidance.

· We don’t handle individual complaints BUT can signpost you in the right direction for complaints and information regarding health and social care services.

· We look for trends within your feedback and produce summary reports of all views received showing the biggest issues raised, to help us decide which areas to prioritise for further research.

· All information will be handled and shared in accordance with General Data Protection Regulation (GDPR) and Data Protection Act 2018. We will take all necessary steps to ensure that your information is treated securely.
