Dear Friend,

Together for Short Lives wants to improve the quality of life and the quality of end of life care for children and young people with life-limiting and life-threatening conditions. We want to ensure that their families are supported, informed and empowered on their journey with their child. And we want to help achieve the best for those individuals and organisations providing vital information, care and support to children and families, whether that be in the community, in hospice or in hospital.

To achieve this we are consulting on what our strategy for 2018 and beyond should be and we are asking for as many views as possible to help shape that strategy.

We want to understand what you think great care and support looks like and what our role could be in helping that happen. 

This short survey is a chance to share your thoughts and ideas with us – we are planning to keep this survey open until 17 May 2017. Your feedback will inform our draft strategy, which you will also have an opportunity to give your views on in the late Summer.
