Returning to work onsite questionnaire for all staff

You will have seen that the government has relaxed shielding measures from 1 August, it is their expectation that staff are able to return to site working from this point as long as social distancing measures are in place and the organisation is Covid-19 secure. The government has stated where providers apply the full measures as advised, the risks to all staff will be mitigated significantly, including those who are clinically extremely vulnerable and clinically vulnerable. The government expects this will allow most staff to return to the workplace. This questionnaire we are asking you to complete includes questions on your current state of health and includes questions for those who are classed as extremely clinically vulnerable or clinically vulnerable. Given all of the measures available, please share with Human Resources if your personal situation means you require further adjustments. Please include what further adjustments you propose.

The government add that people who live with those who are clinically extremely vulnerable or clinically vulnerable can attend the workplace, provided the organisation is COVID-secure.
