Section 1 - About you and your child and young person (CYP)

The Eastern Regional Parent Carer Forums have been supporting NHSE and NHSi to Prevent Inappropriate Admissions to mental health hospitals for children and young people with Special Educational Needs and/or Disabilities (SEND), otherwise called TIER 4 ADMISSION AVOIDANCE. This online survey seeks your views. We aim to use what we learn from all that you share to identify good practice, highlight barriers to access, and identify gaps in service / provision.  This survey is in five sections and is made up of a number of choice responses with a few text boxes to capture your experiences / views. It should take about 20 minutes to complete.

When things are more complex there will be a need to access a range of specialist services and for practitioners to work together in a multidisciplinary way, this level of support is called Tier 3 support. In such cases your child / young person will receive tailored support and may be recorded on the Dynamic Support Register (DSR). This registration will enable different practitioners to carry out different assessments in a joined -up way. Does this apply to your child / young person?
