This survey has been designed to get the views of patients, carers, healthcare professionals, and clinical information system suppliers on draft national standards for the structure and content of outpatient letters.
 
The draft standards have been developed following an evidence review and informed by a consultation workshop held at the Royal College of Physicians on 19 January 2017.
 
The results of this survey will inform the final draft of the outpatient letter headings and definitions. The Professional Record Standards Body will seek endorsement of the final draft from its membership, which is made up of patient and carer, health professional and industry organisations.
