Skin Cancer Nurse Support Service

This is a pilot project to provide essential support to patients with skin cancer attending the Dermatology outpatient clinics. The pilot project is currently running across all ULHT Trust sites, Lincoln, Grantham and Boston Hospitals. The team are evaluating the pilot project and would be grateful if you would complete this questionnaire.
