About this survey
Why are we doing this survey?
We want to understand more about the impact of the Covid-19 (coronavirus) pandemic on local people and their care and support. Our findings will help local decision makers decide how to improve services in the future.
What will happen to the information I give?
This is an anonymous survey so what you tell us will not directly affect your care.
Please only complete the survey if you are happy with us sharing what you tell us with:
- People who design, run and fund health and social care services (Sheffield City Council, Sheffield Clinical Commissioning Group and Sheffield's NHS trusts)
- The Care Quality Commission (CQC) - they inspect health and care services
- Healthwatch England - they are helping central government learn about people's experiences of care and support across the country.
The survey findings will be available to the public on our website: www.healthwatchsheffield.co.uk.
How long will the survey take to complete?
It depends how detailed your answers are and how many experiences you want to tell us about. You can skip sections of the survey that are not relevant to you and any questions that you do not want to answer.
What if I need help filling in the survey or have any questions?
If you would like someone to complete this survey with you over the phone or have any questions please phone us on 0114 2536688 (Monday - Friday, 9:30am - 3:30pm).
We do not give people medical help or support. Please contact NHS 111 with COVID-19 related symptoms or 999 in an emergency. You can contact your GP about any other health concerns you may have.
