'Essential / Critical Worker' Child Information Form

We are deeply saddened that we are unable to open to all our Endeavour Families due to the recent Governement annoucment, but we are dedicated to supporting our local community to stay safe and well.

We appreciate that most families will be able to manage child care whilst schools are closed to fully support social distancing and the containment of the new variant of COVID-19.

Please complete this response form if you believe that you are 'An Essential/ Critical Worker' and require child care support (as defined by Government Guidelines). Please note, where there are two parents or carers in the house-hold, childcare will only be provided if both adults are classed as 'Essential/ Critical Workers'.

If your child has a current EHCP or is supported by a Social Worker we will contact you directly after we have assessed need and space in school. You do not need to complete this form.

You only need to complete one form per family. By helping us to complete this form, you will be providing us with vital information to organise suitable provision in school. Please complete the form with as much detail as possible.

Please do not send your child/ren into school on Tuesday 5th January, 2021.

All applications are subject to approval and parents will be notified by e-mail or telephone if a place has been allocated. In some instances we may require further information.
