Knowledge, Attitude, and Practice of pharmacists in the UK regarding COVID-19

Introduction

This survey contains a list of questions which, in addition to collecting socio-demographic information about yourself, are designed to collect information related to your knowledge of, attitudes towards, and practice during the current COVID-19 outbreak. According to WHO, COVID-19 stands for Coronavirus disease 2019 which is the name of the disease caused by the newly identified coronavirus: SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus-2).

Please take a few minutes to answer all of the questions - the survey will take a maximum of 15 - 20 minutes to complete. Your responses will be anonymous and entirely confidential and will not be used for any purpose other than this research which is being conducted by The Pharmacists’ Defence Association (PDA) and Aston University.

1. I confirm that I have read and understood the Participant Information Sheet (Version 1, dated 07/10/2020). I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily.

I understand that my participation is entirely voluntary and that I am free to withdraw at any time up to the point where I submit my answers, without giving any reason and without my legal rights being affected. I understand that it will not be possible to remove my answers once submitted, because the data is anonymous.
