Hammersmith and Fulham CCG and NHS England are making progress locally with the national review of GPs Primary Medical Services (PMS) contracts. Currently some GP practices under the PMS contract arrangements receive extra funding per patient that is not available to practices that have the General Medical Services (GMS) contracting arrangement.

These arrangements are changing so that all practices will be given the opportunity to provide a number of additional services. Practices who choose to offer the additional services will receive specific funding for them. This will increase the provision of these services to a wider range of patients in Hammersmith and Fulham.

Under these new arrangements a number of key performance indicators will be introduced. There will be two indicators that relate specifically to the "patient voice".

In the question that follows we would like you to indicate which two of these "patient voice indicators" you think we should implement, and therefore ask practices to focus on under the new arrangements.

The five patient voice indicators we are able to choose from are:

·         Overall how would you describe your experience of making an appointment
·         Overall how would you describe your experience of your GP surgery
·         Generally how easy is it to get through to someone at your GP surgery on the phone
·         How convenient was the appointment you were able to get
·         Percentage of patients who would definitely or probably recommend their GP

Once we have received the feedback on these indicators and reached agreement on which two to implement, the monitoring of these will form part of the agreement in place with GP practices who wish to deliver the additional services.
It will take approximately two minutes to respond to this survey.

Thank you for your time and contribution.
