Angus Mental Health Service - People with mental health and substance use problems

Angus Mental Health Service comprises of the 3 Community Mental Health Teams (CMHT) and Angus Integrated Drug & Alcohol Recovery Service (AIDARS).

We are keen to obtain your comments about the service you have been receiving from our staff, as well as suggestions on how we can improve the service.

Please complete this questionnaire by telling us what was good about your experience with the service and what you feel we can improve upon. It will be helpful if you are honest and specific as possible. This is a confidential questionnaire and will be analysed collectively and anonymously by the Service Improvement Adviser. The results will not identify you individually. The results of the analysis will be used for improvement purposes.

We will discuss your comments in order to continuously improve things for existing and future persons who will use the services. At the moment, we have a specific Improvement Project looking at services for people who have both mental health and substance use issues. Your views will be used for this project as well.

This questionnaire was designed with the help of members from Angus Voice as well as members of the Improvement Project Team. Thank you all for your input.

Thank you for completing this questionnaire. Your help is greatly appreciated.
