Angus Mental Health Service - Carers Feedback

Angus Mental Health Service consists of the 3 Community Mental Health Teams (CMHT) and Angus Integrated Drug and Alcohol Recovery Service (AIDARS).

This survey aims to allow those who care for a family member or someone close to them to provide feedback of their experiences when in contact with our services. We are interested in hearing your views, even if your contact has only been limited. 

Please complete this questionnaire by telling us what was good about your experience with the service and what you feel we can improve upon. It will be helpful if you are as honest and specific as possible. The questions should be answered by you - the carer, and your answers should be from your point of view. This is a confidential questionnaire and will be analysed collectively and anonymously by the Service Improvement Adviser. The results will not identify you individually. The results of the analysis will be used to help make improvements to those experiences.

We will discuss your comments in order to continuously improve things for existing and future carers. At the moment, we have a specific Improvement Project looking at services for people who have both mental health and substance use issues at the same time. Your views will be used for this project as well.

This questionnaire was designed with the help of members of the Improvement Project Team, which included representative from Angus Carers. Thank you all for your input.
