Health and Wellbeing Service Engagement Survey, Main study Edited

By participating in the research, you will contribute to the improvement of Swansea Bay University Health Board's services. The questions are mainly about your health and wellbeing during the last 2 years, pandemic, accessing to the health services and CoVid-19 vaccination programme. There are 50 questions and it is predicted to last about 10 minutes according to our pilot study. We will write a report from the project at the end, but all personal data collected during the research will be kept strictly confidential and will only be seen by members of the research team. As required by the Data Protection Act, we will not pass on any person-identifiable data to any external agency.
