Introduction

This survey is to find out what practical support families of children/young people aged 0-25 with additional needs and/or disabilities need at the current time as well as what they think they might need over the next three months.  We will use the anonymised data to demonstrate what families needs are and ask education, health and care services to work with us to try and meet those needs.  No identifiable information will be shared outside of SPCN unless we have asked you and you have given us permission to do so.  We cannot guarantee that all the identified needs will be met, but we will report back with updates on any progress we are making.  Thank you for helping us.
