Why are we asking these questions?
The OR Society is committed to supporting equality, diversity and inclusion in all its activities, and wants to ensure that its services are open to everyone. We are asking these questions so we can better understand the diverse make-up of our event speakers and to help us identify any barriers to inclusion.

We appreciate that you may find some of these questions personal. The form is optional. You can answer as much or as little as you want, or choose not to fill it in at all.

What will we do with this information?
The information collected on this form will be treated confidentially in accordance with the Data Protection Act and will be stored on The OR Society database. Access to this information will be restricted to staff involved in the processing and monitoring of this data. It will be used to provide statistical information only.
