As a part of our admissions process, please complete this form to provide the necessary parental consent required.

Routine health checks in school

Local visits, including places of worship which may include travelling by minibus

The use of toilet wipes if required

National Fruit Scheme - your child will be offered a free piece of fruit /vegetable every day

Nursery Milk Scheme - your child will be offered a carton of milk every day

Join the library - if a book is lost or damaged, it will need to be paid for to enable us to purchase a replacement

Participate in swimming (when your child reaches KS2)

Participate in food tasting (please inform the school of any known allergies)

Photographs / videos being taken in school for use on our school website, school displays, publications and local press

Samples of children's work being published on the school website

Internet Access - I understand that the school will take all reasonable precautions to ensure pupils cannot access inappropriate materials. I understand that the school cannot be held responsible for the nature of content of materials accessed through the Internet. I agree that the school is not liable for any damages arising from use of the Internet facilities.

I give permission for my child to go on school educational trips which may include travel by coach, minibus or public transport.

I give permission for my child to receive emergency dental, medical or surgical treatment including anaesthetic or blood transfusion, as considered necessary by medical authorities present.

I acknowledge the need for my child to behave responsibly during visits.

I accept that unacceptable behaviour before or during any visit may result in my child being withdrawn from activities and that I will be responsible for any costs incurred in returning my child to school.
